Here is some of what you can expect from being a Clinical Operations Specialist with us: You will Interpret orders and transcribe or verifie them accurately and efficiently into the pharmacy computer system You will compound and dispense pharmaceuticals You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose You will provide clinical consultation and clarification to practitioners You will suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed You will sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection You will attend and actively participate in daily multidisciplinary and bedside rounds You will complete drug utilization reviews to promote rational drug therapy and implement improvements identified You will provide accurate and timely medication information to healthcare staff and patients You will lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership. Develop, manage and mentor antimicrobial management program (AMP). You will be a leader and mentor of the antimicrobial management and other clinical programs You will document all clinical activities and interventions accurately and completely You will be responsible for mentoring and education as well as ongoing analysis of interventions You will support and participate in the goals of the organizations quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives. You will participate in the design, collection, and reporting of adverse drug reactions, drug utilization reviews, medication errors and pharmacy quality initiatives You will lead the training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians Develop and document staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified You will lead in preceptorship of pharmacy students and/or residents. You will maintains professional competency sufficient to meet current and future needs of the facility. You will provide and demonstrate expertise in a specialized area of pharmacy practice (operational and/or clinical) You will provide basic technical support for automated dispensing machines and other pharmacy computer systems You will supervise pharmacy operation activities and provides support to the whole pharmacy team You will ensure compliance with facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses and other members of the healthcare team. You will effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions. You will train and participate with Disaster Team drills and real-event response as assigned You will train and participate with Code Teams as assigned. BLS-ACLS or PALS preferred. You will maintain appropriate controlled substance records. Work with the Pharmacy Director appropriate manager to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards. Participate in controlled substance reviews and audits You will comply with all regulatory standards. Work with the Pharmacy Director to identify and develop processes to meet or exceed all regulatory standards. Participate in quality and regulatory reviews and audits You will prioritize work and lead others to comply with facility medication turn-around time policy and procedure. Identify areas for improved efficiency. Assists in implementation of process changes. You will supervise and assist pharmacy technicians and other support staff effectively You will participate in establishing pharmacy goals and objectives and assists in the implementation You will participate with various nursing and medical staff committees as necessary You will assist with staff performance evaluations You will act as manager in charge as assigned You will provide review and guidance on physician order sets and other evidenced based medication use guidelines You will assist with the preparation of the pharmacy newsletter and/or other forms of written and electronic communication You will perform drug class reviews and drug monographs You will participate in all aspects of managing the drug formulary You will assist with the development, implementation, and monitoring of pharmacy operations initiatives (operational/clinical) You will participate in the development and review of pharmacy policies and procedures You will assist in the design and update of pharmacy computer system programs relating to pharmacy, nursing and physician programs You will assist in the maintenance of pharmacy dictionaries including adding new drugs to the drug dictionary and Gold Standard formulary Charge Master maintenance Qualifications you will need for this role: 4 year Bachelor Degree; PharmD preferred Co Pharmacist Licensure Experience required-1 of the following 1 year as an Advanced Clinical Pharmacist or at least 5 years Hospital experience required or Completion of hospital PGY2/pharmacy specialty residency (or greater) or specialty BCPS certification or other certification (FCCP, FASHP) 3-5 years total Pharmacist experience required Hospital experience preferred Sky Ridge Medical Center, the first hospital to open in Douglas County in 2003, is a Level II Trauma Center and an acute care hospital with 284 licensed beds.
